CCNC's medical home approach is continually being refined and improved. We are also moving forward with new approaches to care, often on a pilot basis while we determine what works best for patients and providers.
Originally focused solely on improving care and lowering costs for the state's Medicaid population, CCNC is increasingly working with other payers, including Medicare, Blue Cross and Blue Shield of North Carolina, and North Carolina's State Health Plan. Our long-term goal is to bring the benefits of the medical home model, including coordinated care management and robust data analysis, to all of North Carolina's citizens.
