Case Management Information System
Coordinating patient care
The Case Management Information System (CMIS) is a user-built, patient-centric, electronic record of care management activities used by CCNC care managers since 2001, with over 1,500 active users statewide. CMIS contains demographic data and claims data on over 2.8 million Medicaid recipients, of whom approximately 1 million are currently enrolled with a practice in a CCNC network.
CMIS also contains enrollment, eligibility and case management services statewide for HealthNet projects, which are regional collaboratives providing care to the uninsured. HealthNet currently serves 24,000 enrolled individuals. Patients enrolled in Medicaid and HealthNet reap the benefits of the continuity of care provided by CMIS. The system maintains a health record and single care plan that stays with the patient as he or she moves from one area of the state to another or across eligibility programs. CMIS contains standardized health assessments, care plans, screening tools, disease management, health coaching modules, and workflow management features.
