AccessCare AccessCare serves 24 North Carolina counties.
AccessCare is the largest of the 14 CCNC networks comprised of primary care physicians committed to providing the highest quality medical care for the Medicaid, Medicare, and uninsured populations of North Carolina. We collaborate with the primary care provider, Department of Social Services, Mental Health, and other community agencies to ensure prevention, treatment and continuity of care.
Established in July 1998 as a member-owned, not-for-profit organization of 23 practices, AccessCare has grown to a statewide network of over 300 primary care practices with 1,000 providers caring for over 260,000 Medicaid enrollees as of December 2010.
While most CCNC networks serve a contiguous, multi-county region, AccessCare serves patients in 23 counties spread across North Carolina: Alamance, Alexander, Alleghany, Ashe, Avery, Burke, Caldwell, Caswell, Catawba, Chatham, Cherokee, Clay, Graham, Haywood, Iredell, Jackson, Macon, Orange, Robeson, Sampson, Swain, Watauga and Wayne. AccessCare also manages five specific care locations -- or "sites" -- in Eastern and Central North Carolina. (See network entry below.)

AccessCare Network Sites AccessCare manages five sites.
While most CCNC networks serve a contiguous, multi-county region, five specific locations -- or "sites" -- are managed by AccessCare, a CCNC network with its main office in Morrisville, NC. Four of these sites are in Eastern North Carolina and one is in Davidson County. These sites are represented by gray circles on the map at the top of this page.

Community Care of Western North Carolina Serving Buncombe, Henderson, Madison, McDowell, Mitchell, Polk, Transylvania, and Yancey Counties.
CCWNC is the CCNC network for eight counties in Western North Carolina. Established in 1998 as a local 501(c)(3) organization, CCWNC started as one of 14 Community Care networks serving Carolina Access Medicaid patients. Our population is a mixture of rural and urban counties with patients across the life span. CCWNC is designed to improve access to and quality of healthcare services. We are committed to assuring integrated healthcare and enjoy a strong collaborative relationship with Local Management Entities for mental health services in WNC. Over the past 13 years, our scope has expanded to include services for uninsured adults through HealthNet, dually eligible beneficiaries’ via a CMS 646 federal waiver and we will soon add a multi-payer project for the commercially insured population. The network offers care management programs and services to maximize access to health care and to improve its quality and cost-effectiveness for all populations served.

Community Care of the Lower Cape Fear Serving Bladen, Brunswick, Columbus, New Hanover, Onslow and Pender Counties.
Launched in 2003, Community Care of the Lower Cape Fear (CCLCF) serves the urban center of Wilmington, NC and surrounding rural communities in Bladen, Brunswick, Columbus, New Hanover, Onslow and Pender Counties. CCLCF works in close collaboration with the physicians, hospitals, county health departments and departments of social services in our six-county network to improve health outcomes for nearly 60,000 eligible patients. Our 137 provider practices span from one of the very largest multi-specialty practices in the state to many single-provider, rural practices.
CCLCF is fortunate to participate in the federal CMS 646 initiative to care manage dually-eligible Medicaid and Medicare patients in New Hanover and Pender Counties and has been selected to participate in a multi-payer demonstration that will extend our services to Medicare, Blue Cross, and State Employee Health Plan patients in Bladen and Columbus Counties.

Carolina Collaborative Community Care Serving Cumberland County.
Carolina Collaborative Community Care, Inc. (4C) is a single-county network with approximately 48,000 enrollees, 84 primary care practices, one large health system and 20 care managers. 4C serves
Cumberland County, the home of Fort Bragg, the largest military installation in the United States. The area has a very diverse and mobile population.
4C’s working relationships with local organizations including the health department, department of social services, health system and local management entity help provide effective services to the Medicaid population.The Numbers for our network as of Feb 2011:

Carolina Community Health Partnership Serving Rutherford and Cleveland Counties.
The Carolina Community Health Partnership Network serves the two rural counties: Cleveland and Rutherford. Of the 23,808 Medicaid enrollees served, almost half are children. There are two small community hospitals that have inpatient psychiatric admission units. The Network has a large population of enrollees diagnosed with Diabetes and Chronic Obstructive Pulmonary Disease. Of the 4090 enrollees in the Aged, Blind and Disabled category of Medicaid, 52% have a mental health diagnosis and two of our network’s primary care practices have a co-located mental health professional – one serving children and one serving adults.

Community Care of Wake/Johnston Counties Serving Wake and Johnston Counties.
Community Care of Wake and Johnston Counties encompasses two very different counties. Wake is a highly populated, largely urban county that contains our state capitol. Johnston County is a geographically large, rural county. Overall, our network has a high percentage of pediatric patients, both within our general population and our disabled population. We have a culturally and linguistically diverse population, with a high percentage of Spanish speaking families. Our network is made strong by active participation among our primary care providers, hospitals, public health agencies, mental health providers, social service agencies, and community partners.

Community Care Partners of Greater Mecklenburg Serving Anson, Mecklenburg and Union Counties.
Community Care Partners of Greater Mecklenburg is a network of comprised of Anson, Mecklenburg and Union counties, representing a diverse demographic of very rural to very urban. We work with a range of hospitals from a large teaching hospital to a small community-based hospital.
In addition to participating in the 646 waiver, we also have an ED Navigator grant as well as a HealthNet grant. Our enrolled population consists of approximately 60 percent children and includes a significant Hispanic population. Six members of our staff are bilingual to serve them.

Community Care Plan of Eastern Carolina Serving 27 counties in Eastern North Carolina (see list below)
Community Care Plan of Eastern Carolina is geographically the largest of 14 member networks forming Community Care of North Carolina. Covering 27 counties in Eastern North Carolina, most of the counties we serve are rural and many have long-standing access-to-care issues.

Community Health Partners Serving Gaston and Lincoln Counties.
Community Health Partners (CHP) started in July 1998 as the community based Medicaid managed care entity in Gaston. It expanded into Lincoln County in 2001. CHP has expanded over the years to 47 primary care practices serving over 33,000 patients including 5,800 aged and/or disabled. CHP works collaboratively with local health departments, departments of social services and many other community organizations, including area hospitals. CHP provides new member services, care management for high risk patients, disease management, utilization management, pharmacy programs, patient education, transitional care (hospital to home), a palliative care initiative, telehealth, early intervention awareness program, behavioral health integration and a high-risk pregnancy program.

Northern Piedmont Community Care Serving Durham, Franklin, Granville, Person, Vance and Warren Counties.
Northern Piedmont Community Care (NPCC) is a partnership between Durham Community Health Network (for Durham county) and Community Care Partners (for Vance, Warren, Person, Franklin and Granville counties) to promote wellness strengthen the self-care capacity of its Medicaid members and their families. With an interdisciplinary staff of professional and paraprofessional providers, NPCC has focused its efforts and energies on the development of community-based care/disease management that supports the integration and collaboration of the various patient delivery systems within the community. The staff of NPCC deliver at-risk social work services, service coordination, access support, education and nutrition counseling.

Northwest Community Care Network Serving Davie, Davidson, Forsyth, Stokes, Surry, Wilkes, and Yadkin Counties.
Northwest Community Care Network (NCCN) began in 1998 with Surry as the original pilot county. NCCN began to add additional counties in December 2001 and has grown to cover Carolina Access recipients in the counties of Davidson, Davie, Forsyth, Stokes, Surry, Wilkes, and Yadkin. The program as developed in these counties serves both rural and urban settings. In order to provide the necessary managed care services to these recipients the network has affiliated with the health departments, hospitals, and social service departments in each county.
The NCCN Mission Statement reads "We are an organized community network of healthcare providers and care managers, which partners with other community resources to improve the health and well-being of our Medicaid patients in an appropriate and cost-effective manner.”

Partnership for Community Care Serving Guilford, Randolph and Rockingham Counties.
Partnership for Community Care (P4CC) is responsible for the support of Medicaid patients in Guilford, Rockingham and Randolph counties. We presently support 70% of all Carolina Access patients in all three counties. Our purpose is to identify and minimize barriers to healthcare by helping primary care providers in our network to better utilize evidence-based disease state management guidelines. We also provide Care Management services to patients to reinforce what their provider's treatment plan is and to better manage their chronic conditions. These services are provided by medical and behavioral health social workers, nurses, pharmacists and a nutritionist. We also provide additional support to providers of children through our four Health Check Coordinators and our Early Intervention Specialist. Currently we have approximately 85,000 enrollees in our network.

Community Care of the Sandhills Serving Harnett, Hoke, Lee, Montgomery, Moore, Richmond, and Scotland Counties.
Community Care of the Sandhills (CCS) is a non-profit organization that was formed as a 501 c3 in May, 2003 to provide care management services to the Carolina Access Medicaid recipients in Harnett, Hoke, Lee, Montgomery, Moore, Richmond, and Scotland Counties. CCS is comprised of 250 providers at 92 practice sites in a rural seven county area. Our major functions are education of providers and patients and care management of high-risk patients. We employ 25 care managers whose work is managed, monitored, and recorded electronically. We are an intervention network with CCNC/646 Waiver.
The mission of CCS is to be a dynamic organization which provides education, advocacy, and coordination in collaboration with primary care providers and our community. We serve Carolina Access Medicaid recipients and others with limited resources and chronic diseases. We strive to improve the quality and availability of preventive and proactive healthcare in a cost-effective manner.

Community Care of Southern Piedmont Serving, Cabarrus, Rowan and Stanly Counties.
Located at 845 Church Street North in Concord, North Carolina, Community Care of Southern Piedmont serves the surrounding three-county area.
CC of SP is also one of only 17 sites nationally to be designated a Beacon Community, a federal grant program to promote the development of secure, private, and accurate systems electronic health records (EHRs) and health information exchanges. The Beacon Program aims to strengthen health information technology (health IT) infrastructure to improve care coordination, increase the quality of care, and slow the growth of health care spending.
