A focus on high-cost diseases that significant impact quality of life
While the health challenges of any individual patient are unique, the prevalence of some diseases requires a coordinated, system-wide approach to care management. Diseases targeted by CCNC include asthma, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD) and diabetes.
